Qualifications

• Bachelor's degree required in HIV/AIDS, behavioral health, substance use, or related field.
• At least two years of field experience in HIV/AIDS or behavioral health.
• Valid NYS driver's license and insured vehicle for client transport.
• Sensitivity toward cultural diversity and working with people living with HIV/AIDS.

Responsibilities

• Conducts initial patient assessment and develops treatment/service plan
• Provides case management and links to medical, housing, social, and legal services
• Engages with patients with weekly contact to support goal achievement
• Collaborates with team to ensure patient needs are met
• Provides health education on HIV transmission and harm reduction
• Attends case conferences with other providers
