Job summary

Evergreen Health in Buffalo, NY is seeking a Case Manager to join our team in a regular full-time role at 200 S. Elmwood Ave.

The position focuses on harm reduction, linkage to treatment, health education, and helping clients manage care across Western New York. Responsibilities include initial assessments, developing treatment plans, arranging service linkages, advocating for clients, and coordinating with other providers.

Qualifications

• Bachelor's degree and two years of field experience in HIV/AIDS, behavioral health, substance use or other chronic illness
• Valid NYS driver's license and insured, dependable car for patient activities
• Sensitivity toward cultural diversity and working with diverse populations essential.

Responsibilities

• Conducts initial patient assessment and develops treatment/service plan
• Provides case management and arranges service linkages (medical, housing, social, financial)
• Provides health education related to HIV and harm reduction
• Maintains weekly contact with patients to support goal achievement
• Advocates for services and coordinates with other providers
• Attends case conferences and collaborates with team members
