Job summary

Evergreen Health is seeking a Case Manager in Buffalo, NY for a Regular Full-time role. You will coordinate care across the Western New York region, focusing on linkage to treatment, health education and self-management, with frequent contact to support clients in staying in care.

The position requires a Bachelor's degree and two years of field experience in HIV/AIDS, behavioral health, or related chronic illness; a NYS driver's license is preferred for transportation to programs and services.

Qualifications

• Conducts initial patient assessment and develops treatment/service plan
• Engages with and builds rapport with patients, establishing weekly contact toward goals
• Provides case management by arranging service linkages (medical, legal, housing, social, financial)
• Advocates for services that support self-sufficiency and self-management
• Attends/arranges case conferences with other service providers
• Provides health education to clients regarding HIV transmission and harm reduction
• Collaborates with team to meet patient needs

Responsibilities

• Conducts initial patient assessment and develops treatment/service plan
• Conducts health education screening and develops health education plans
• Engages with and builds rapport with patients, establishing weekly contact to assist patients with action steps toward reaching their goals
• Provides case management for clients by arranging appropriate and necessary service linkages (e.g. medical, legal, housing, social, financial)
• Provides support and linkages to clients based on psychosocial assessment and referrals
• Advocates for services that support self-sufficiency and self-management
• Attends/arranges case conferences with other service providers
• Provides health education to clients regarding HIV transmission and harm reduction
• Collaborates with team members to meet patient needs
• Other duties as requested
