Job summary

Metro Healthy Communities in Port Richey, FL seeks a Medical Case Manager to provide patient-centered case management for individuals living with HIV/AIDS, linking clients to health care, psychosocial and supportive services.

The role involves assessment, care planning, coordination of medical treatments, adherence support, documentation, and advocacy to improve access to Medicaid, Medicare and other assistance programs within the local health system.

Qualifications

• Bachelor's degree in human services or related field.
• Minimum of 1 year experience in a social service setting or case management.
• Experience working in HIV/AIDS field preferred.
• English proficiency; Spanish language skills highly preferred.

Responsibilities

• Initial assessment of service needs.
• Development of an individualized service plan.
• Coordinate and follow up on services to implement the plan and medical treatments.
• Monitor client progress and adjust care plans as needed (at least every 6 months).
• Provide treatment adherence counseling and support for complex HIV treatments.
• Collaborate with medical providers and community partners to promote adherence.
• Document activity accurately through case notes and reports.
• Advocate for clients to access Medicaid, Medicare, and other programs.
• Assist clients with referrals to food, transportation, and emergency financial assistance.
