Qualifications

Using a harm reduction approach in a “stages of change” model, theCase Managerwill work with clients to assess barriers and establish goals with a focus on linkage to treatment, health education and achieving self-management. Frequent contact and follow-up with clients will support them in staying in care and decreasing health-related crises. The Case Manager will focus on all seven counties of Western New York, including Erie, Niagara, Orleans, Chautauqua, Wyoming, Cattaraugus, and Allegany county. The Case Manager will provide health education to PLWHA throughout the region, including individual and group education on topics related to treatment options, viral suppression and skills needed for self-care. The Case Manager will, using a structured curriculum in group and individual activities, engage participants in programming that will support behavior change.

As part of the essential functions of this role, the Case Manager:

• Conducts initial patient assessment and develops treatment/service plan
• Conducts health education screening and develops health education plans
• Engages with and builds rapport with patients, establishing weekly contact to assist patients with action steps toward reaching their goals
• Provides case management for clients by arranging appropriate and necessary service linkages (e.g. medical, legal, housing, social, financial); represents client when advocacy is necessary
• Provides support and linkages to clients based on case management psychosocial assessment, addressing counseling needs and making appropriate behavioral health referrals
• Advocates for services that support self-sufficiency and self-management
• Attends/arranges case conferences with other service providers
• Provides health education to clients regarding HIV transmission and harm reduction
• Assists in the planning and implementation of health education groups.
• Collaborates with team members to make sure patient needs are being met
• Other duties as requested

Qualified candidate will have a Bachelor's degree with two (2) years of field experience in HIV/AIDS, behavioral health, substance use or other chronic illness; have and maintain a valid NYS driver's license (verified through NYS LENS system) and insured, dependable car to use for patient/program activities, including transporting patients when necessary;Sensitivity toward cultural diversity, people living with HIV/AIDS, and lifestyle and addiction issues essential.
