Job summary

Meharry Medical College seeks a Medical Case Manager to coordinate comprehensive patient care plans under clinical supervision. The role works as a clinic liaison with patients, EIS workers, Community Health Workers, medical providers and various agencies to keep patients engaged in HIV care.

Responsibilities include assessing care plans, real-time data entry using CAREWare, adherence counseling, patient education, and coordinating transitions for hospitalized or incarcerated clients.

Qualifications

• Master’s degree in a health-related field is preferred; or Bachelor’s degree with a minimum of two years of directly related work experience.
• Experience working in a multidisciplinary team is desirable.
• Familiarity with patients living with HIV is highly desirable.
• Knowledge of Ryan White Standards of Care and clinical workflows is beneficial.

Responsibilities

• Coordinate medical case management assessment and re-assessment.
• Provide direct patient follow-up care and submit required reports.
• Provide adherence counseling and treatment planning.
• Educate patients about HIV/AIDS, STDs, TB and healthy lifestyles.
• Link and re-engage clients to care and coordinate with providers and community agencies.
• Serve as liaison between patients, medical providers and community partners.
