Qualifications

• License and certification requirements to be obtained within specified timeframes.

Responsibilities

• Provide medical case management oversight and adherence to medical protocols.
• Assist clients in accessing primary care, medications, and health insurance.
• Identify and remove barriers to care and ensure adherence to prescribed treatment.
