Responsibilities

• Scans, indexes, and files medical records into the EMR accurately and timely.
• Reviews incoming documents for proper patient identification, completeness, and correct document type.
• Processes release of information (ROI) requests in compliance with HIPAA, state law, and policy.
• Verifies authorization forms and patient identity prior to releasing protected health information.
• Maintains confidentiality and security of all medical records and patient information.
• Tracks and documents requests for records and releases per procedures.
• Responds to requests from patients, providers, attorneys, and insurers within established timeframes.
• Performs quality checks to ensure scanned documents are legible and correctly indexed.
