Job summary

EMPACT-SPC is seeking a High Needs Case Manager to deliver direct services to children and families in home, school, and community settings. You will coordinate crisis intervention, transportation, and family education while maintaining a high level of service intensity with a caseload of 15 families.

The role requires behavioural health experience, a BHT‑level credential or higher, and the ability to work flexible hours including evenings or weekends when needed. Bilingual Spanish is a plus.

Qualifications

• Must meet BHT requirements with a bachelor’s degree or higher and one year of behavioural health experience OR an associate’s degree and two years of behavioural health experience OR a high school diploma AND four years of behavioural health experience.
• Behavioural health experience is required.
• Previous experience working with children is preferred.

Responsibilities

• Provide direct in‑home, in‑office, in‑school, and community services. These services include crisis intervention, transportation, maintaining client stabilization, additional client supervision, parent training in child care and behaviour management, and support to keep the child at home.
• Facilitate, schedule, and provide follow‑up on family, youth, and adult team meetings; document outcomes and share copies with team members.
• Deliver at least 40 hours per week with 30 hours of direct client contact and complete monthly/annual documentation.
• In collaboration with each child and family, support services to meet the 12 Arizona Principles.
• Coordinate with Clinical Coordinator and/or Director for communication with other departments and external partners.
• Develop and maintain effective working relationships with related states, community mental health, and contracting organizations.
