Job summary

Community Counseling Services is seeking a Case Manager to provide clinical, direct services for the IMPACT program serving adults with severe and persistent mental illnesses and their families.

This is a rotating weekend position with an on-call crisis component and a 40-hour week for a full-time provider. The role emphasizes intake, assessments, treatment planning, and family collaboration within a multidisciplinary team.

Qualifications

• The Case Manager shall have at least an associate’s degree; a bachelor’s degree is preferred in the social sciences.
• Experience working with children, families, and/or vulnerable populations is preferred.
• Skills, knowledge, and/or experience demonstrating care for individuals with severe and persistent illnesses.

Responsibilities

• Provide case management services to consumers and families.
• Conduct client intake, assessments and diagnostic interviews.
• Develop and update treatment plans in a timely manner.
• Complete progress reports and discharge summaries.
• Offer on-call crisis intervention on a rotating schedule.
• Assist with daily living activities and linkage to services.
• Collaborate with families, treatment teams and community resources.
