The role collaborates with healthcare providers to ensure fidelity to the model, coordinate evaluation, and oversee data reporting while expanding access to community resources for families.

Qualifications

• Bachelor’s degree in Child Development, Social Work, or a closely related field.
• Excellent written and verbal communication skills.

• Experience supervising staff and coordinating services for children.

Responsibilities

• Oversee and support case management staff with orientation and ongoing training.
• Coordinate data collection and reporting to meet program outcomes.
• Screen children for developmental delays using ASQ/ASQ:SE and review referrals.
• Manage referrals and liaise with local programs and providers.
• Maintain confidential client records per HIPAA.
• Work with partners to ensure services match families’ needs.
