Job summary

Paylocity seeks a Supervisor - Help Me Grow to coordinate and expand the HMG system in Merced County. You will lead case management staff, ensure fidelity to the model, and support families in accessing services and resources.

You will work with partners to collect data, report outcomes, and maintain high-quality documentation while upholding HIPAA and confidentiality standards. Flexibility for non-traditional hours is expected to meet family needs.

Qualifications

• Bachelor’s in Child Development, Social Work, or closely related field.
• 3+ years of experience providing care coordination services to children with special needs and their families.
• 2+ years of supervising, motivating, and training staff to achieve program goals.

Responsibilities

• Oversee and support case management staff, provide orientation and ongoing training, and monitor performance to ensure program outcomes.
• Collaborate with Help Me Grow implementation partners to collect data for reporting requirements across Merced County.
• Screen children for developmental delays using ASQ/ASQ:SE and review referral options with parents.
• Coordinate referrals and act as liaison between staff and local programs and providers.
• Accurately collect, enter and submit all necessary program documentation and data information.
• Maintain confidential client records in compliance with HIPAA and related regulations.
