Job summary

Exceptional Parents Unlimited is seeking an Outreach Coordinator to engage communities, present Help Me Grow services, and educate families about ASQ screenings. The role collaborates with the HMG Case Manager and Supervisor to connect families to local resources in the Merced County area.

The position requires coordinating outreach events, maintaining a caseload, and promoting partnerships while ensuring confidentiality and HIPAA compliance. Some local travel and flexible hours may be necessary.

Qualifications

• Education: High School Diploma; associate degree preferred in social work, early childhood development, or related field.
• Experience: 2+ years working with families and children; case management preferred.

Responsibilities

• Participate in outreach events and connect with community partners.
• Educate community about Help Me Grow services and ASQ screenings.
• Maintain a caseload of 15-20 active families in the county.
• Prepare meeting minutes and coordinate coalition meetings.
• Enter and maintain client data in the Apricot database.
• Travel locally with a valid CA driver’s license as needed.
