Job summary

Baptist Health seeks an RN Case Manager to oversee assigned caseloads, coordinate assessments, planning, and discharge planning, ensuring optimal resource use and outcomes. The role emphasizes heart failure patient care, collaboration with the Transition of Care Pharmacist, Heart Failure Clinic and RPM.

The position requires a Kentucky RN license, BSN (or equivalent experience), and 3 years in case management with certification within two years of hire.

Qualifications

• BSN degree or combination of education and clinical experience may be considered
• Current unrestricted RN license in Kentucky required
• Three years of job-related experience required
• Certification in Case Management (ANCC/ CCM/ ACM-RN) or other approved certification preferred and must be obtained within two years

Responsibilities

• Coordinate and complete timely assessment, planning, implementation and evaluation of discharge plans and care transitions
• Coordinate communication among care providers internal and external to the health system
• Identify patients who would benefit from case management and manage activities across assessment, planning, delivery, monitoring and evaluation
• Focus on heart failure inpatients for disease-specific case management and collaborate with Transition of Care Pharmacist, Heart Failure Clinic and Remote Patient Monitoring
