Job summary

Qualifications

• Must possess, as a minimum, a high school diploma or its equivalent.
• Associate or Bachelor’s Degree preferred but not required.
• Prior health-care experience preferred.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Develop and coordinate department policies and procedures and review them annually.
• Identify medically related social and emotional needs of residents and contribute to treatment planning.
• Coordinate social service activities with other departments and community agencies.
• Assist in discharge planning and arranging outpatient services when needed.
• Maintain regular social service progress notes and report to the Director.
• Provide information on Medicare/Medicaid and financial assistance programs.
