Job summary

North Shore Healthcare is seeking a Social Services professional to provide residents with comprehensive social work support. You will evaluate resident needs, coordinate services with families and an interdisciplinary team, and manage case coordination to support care plans.

The role emphasizes documentation, community resource linking, and training responsibilities, with opportunities for growth in a supportive skilled nursing environment.

Qualifications

• Excellent written and verbal communication skills.
• Computer proficiency with Microsoft products.
• Current state license as a Social Worker preferred.
• Bachelor Degree in social work or social welfare.
• Experience in outpatient rehabilitation preferred.
• Experience in a fast-paced healthcare environment preferred.
• Post-acute care or health care experience preferred.

Responsibilities

• Provide social services to residents as specified in care plans and aligned with goals.
• Coordinate social services with residents, families, staff and others.
• Perform case management or case coordination as needed for residents.
• Contact and coordinate resident utilization of community resources.
• Maintain timely and comprehensive documentation of resident status and services.
• Provide specialized training to staff, family members, consultants and others as needed.
• Participate in clinical conferences and in-service programs.
• Comply with laws and the Center's Compliance Program.
