FOREST GROVE POST ACUTE seeks an experienced nurse/case manager to develop and coordinate care plans for residents, balancing quality care with cost considerations. You will work with residents, families, and healthcare providers to ensure timely, appropriate services.

The role emphasizes charting, admissions and discharges, and utilization review, with opportunities to apply PCC knowledge and Medicare/Medicaid insights in a collaborative, high‑volume environment.

Qualifications

• Bachelor's Degree in Nursing or Social Work.
• RN license preferred.
• LVN/LPN license acceptable.
• 2 years clinical nursing experience preferred.
• Knowledge of Medicare, Medi‑Cal and Medicaid programs and benefits.
• PCC Knowledge preferred.
• CCM certification a plus.
• Continuing education/licensing required.

Responsibilities

• Develop and implement care plans for residents.
• Communicate plans with residents, families, and team.
• Coordinate admissions and discharges.
• Perform utilization review and cost‑effective care.
• Maintain accurate documentation and chart reviews.
• Coordinate referrals with payers and care partners.
