Job summary

Qualifications

• Must possess, at minimum, a high school diploma or its equivalent.
• Associate or Bachelor’s Degree preferred but not required.
• Prior health-care experience preferred.

Responsibilities

• Assist Social Service Director in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff and related departments in planning social services.
• Assist in development, administering, and coordinating department policies and procedures.
• Review department policies at least annually and participate in changes.
• Assist in identifying medically related social and emotional needs of residents.
• Coordinate social service activities with other departments as necessary.
• Provide information to residents and families about Medicare/Medicaid and other financial assistance programs.
• Record and maintain Social Service progress notes indicating response to treatment plans.
