Job summary

FOREST GROVE POST ACUTE in Forest Grove, OR is seeking a Resident Care Manager to oversee clinical coordination and plan-of-care development for residents. You will collaborate with families, healthcare providers, and payers to ensure high-quality, cost-effective care and effective communication across teams.

The role requires nursing education, licensure considerations, and experience in a post-acute setting; strong documentation and organizational skills are essential.

Qualifications

• Bachelor's Degree in Nursing or Social Work required or preferred
• RN license preferred; LVN/LPN acceptable
• Two years clinical nursing experience preferred
• Knowledge of Medicare, Medi-Cal and Medicaid programs and benefits
• PCC Knowledge preferred
• CCM certification a plus
• Must maintain continuing education/licensing
• Good standing with the Department of Public Health, License and Certification Division

Responsibilities

• Communicate regularly with residents and family about Plans of Care and treatments
• Participate in morning meetings to facilitate team communications
• Address questions about care and treatment plans to residents and families
• Monitor and adjust residents' statuses based on changing needs
• Organize daily work and prioritize care for residents
• Complete required documentation and charting
• Perform utilization review for cost-effective care
• Coordinate care with residents, providers, and payers
• Oversee admissions and discharge activities
• Coordinate referrals to and from the facility
