Responsibilities include coordinating with physician orders, ensuring timely communication, and maintaining equipment and data entry while adhering to policy. This role emphasizes service excellence and teamwork in a community-focused setting.

Qualifications

• High School graduate or equivalent.
• Ability to operate a personal computer.
• Ability to handle telephone calls with proper etiquette.
• Ability to effectively communicate with a variety of professional and lay people.
• Ability to use personal computer, printer, copier, fax machine, 10 key calculator.
• Organized in work habits.
• Punctual attendance at assigned work location.

Responsibilities

• Answer telephone lines promptly and efficiently; respond to messages appropriately.
• Manage visit calendars and update physician orders; audit visit notes.
• Handle Resumption of Care (ROC) processes and recertification triggers.
• Coordinate with Home Health Liaison regarding patient admissions and hospital stays.
• Enter daily visits data; maintain safety and policy compliance.
• Participate in department projects and required trainings.
