Job summary

Firelands Health - Home Health is seeking a licensed RN to perform home care visits in Sandusky and surrounding communities, delivering skilled nursing care per the Plan of Care and reporting to the RN Clinical Supervisor.

Autonomy in your role, a schedule you design, and a supportive, employee-focused culture with opportunities to grow. A valid Ohio RN license, BLS/CPR, and a driver’s license are required.

Qualifications

• Graduate of an accredited nursing program with active Ohio RN license.
• BSN preferred or higher education is a plus.
• Experience with hospice or home health is preferred.
• Strong communication and critical thinking skills.
• Ability to work independently and manage care plans.

Responsibilities

• Performs home care visits after the initial assessment and updates the Plan of Care.
• Delivers skilled nursing care in the patient’s home as ordered by the physician.
• Executes autonomous decisions using clinical training and patient knowledge.
• Completes timely documentation and patient education records.
• Communicates with physicians, supervisors, and co‑workers promptly.
• Promotes continuity of care during admissions, transfers, and charges.
• Performs other duties as assigned.

What You Will Do:

• Performs home care visits after the initial assessment, collects on going data, and reports to the RN Clinical Supervisor, implementing the POC.
• Provides skilled nursing care in the patients home as ordered by the attending physician in conjunction with the Plan of Care.
• Autonomy within your position, use your training, critical thinking and knowledge of your patients to make decisions
• Submits clear and appropriate documentation in a timely manner and documents patient education.
• Communicates effectively to physicians, supervisors, and co-workers in a timely manner.
• Promotes continuity of care with appropriate admissions, transfers, and charges.
• Other duties as assigned.

Skills:

• Effective communication skills (verbal and written)
• Experience with Electronic Medical Records (EMR) software is preferred
• Time management and Organizational skills
• Demonstrated ability to work autonomously as a registered nurse
• Excellent planning, organizing and communicating.

What you will need:

• Graduate of an accredited school of professional nursing, BSN preferred, currently licensed as an RN through the Ohio State Board of Nursing.
• Prior Nursing experience in Hospice or Home Health setting preferred. At least one year's medical/surgical nursing experience preferred.
• BLS/CPR Certification required
• Must possess a valid State of Ohio motor vehicle operator's license and be insurable under Firelands' auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.
• Independent decision‑making skills and abilities to render nursing care in an independent setting.
• Ability to rotate "On Call" after hours, weekends and holidays.
