Job summary

Enhabit Home Health & Hospice is seeking a dedicated professional to manage relationships with physicians, hospitals, and other referral sources. You will promote awareness of Enhabit services and guide patients through post-acute care transitions to improve outcomes.

This role requires a Bachelor's degree in healthcare administration or related field, 2+ years in healthcare business development, strong communication and presentation skills, and a valid driver's license.

Qualifications

• Bachelor's degree in healthcare administration, marketing, or related field or graduate of nursing/therapy/social work.
• Highly preferred graduate of nursing, therapy or social work.
• Minimum two years in healthcare business development, community relations, marketing, or sales.
• Experience with federal/state/local laws for Medicare home health operations.
• Must have and maintain a valid state driver's license and auto liability insurance.
• Strong interpersonal, communication and presentation skills.
• If clinical: must maintain applicable clinical license.
• Preferred experience in home health, hospice or post-acute care.
• Preferred established referral relationships with physicians, hospitals, or senior communities.
• Must have dependable transportation and English fluency; MS Office proficiency.
• In-office attendance is essential to meet objectives.

Responsibilities

• Serves as the primary relationship manager between Enhabit and referral sources including physicians, hospitals, and health systems.
• Supports referral growth by educating sources about services and program requirements to navigate post-acute care transitions.
