You will partner with clinical and support teams to implement evidence-based changes and monitor outcomes. The role emphasizes process improvement, change management, and NCQA-aligned initiatives to elevate care quality across departments.

Qualifications

• Bachelor's degree in health-related field or industrial engineering.
• Master's degree preferred or equivalent work experience.
• Four years of healthcare experience with minimum three years in quality improvement, process improvement, change management, etc.

Responsibilities

• Evaluate performance through assessment of workflow efficiency, staff competency, resource utilization, safety and compliance.
• Lead improvement plans following the IHI Model for Improvement and LEAN.
• Collaborate with departments to identify and prioritize opportunities for improvement.
• Analyze data for patterns and trends in care delivery; identify root causes of issues and critical events.
• Disseminate and maintain trend reports; translate findings into actionable improvement strategies.
• Ensure staff education and resources to facilitate changes.
• Collaborate to enhance NCQA recognition for PCMH, Diabetes, Heart and Stroke.
