Job summary

Family Circle of Care in Tyler, TX is seeking a detail-oriented Clinical Quality Improvement Analyst to collect, analyze, and report on clinical data, supporting quality performance goals. This on-site role collaborates with clinical and operational teams to drive improvements in patient outcomes and care quality.

The ideal candidate has a background in healthcare data analytics, experience with EHR systems (Epic preferred), and strong Excel skills to create dashboards and scorecards for ongoing

Qualifications

• Bachelor's degree or equivalent combination of education and relevant experience.
• At least 2 years of healthcare quality improvement or related experience.
• Experience with electronic health record systems; Epic preferred.
• Strong Excel skills including formulas, pivot tables, and data analysis.
• Knowledge of clinical quality measures, healthcare reporting, and quality improvement processes.

Responsibilities

• Clinical Data Analysis & Reporting: Collect, validate, and analyze clinical data from our EHR, payer reports, and other healthcare systems.
• Develop and maintain dashboards, reports, and scorecards to track clinical quality measures and organizational performance.
• Identify trends, gaps in care, and opportunities to improve patient outcomes.
• Collaborate with clinical and IT teams to resolve data discrepancies and improve reporting accuracy.
• Present findings and recommendations to clinical teams and leadership.
• Quality Programs & Value-Based Care: Coordinate payer incentive programs and value-based care initiatives, including performance goals, reporting requirements, and deadlines.
• Monitor quality measures, preventive care, chronic disease management, and other performance indicators.
• Work with clinical teams to develop strategies to close gaps in care and improve quality outcomes.
• Track incentive program performance and collaborate with finance and billing teams to reconcile payments.
• UDS Reporting & PCMH: Coordinate Uniform Data System reporting and PCMH recognition activities.
• Collect, review, and validate information needed for required submissions.
• Maintain documentation and monitor compliance with reporting requirements and program standards.
• Partner with departments across the organization to support accurate reporting and continuous improvement.
• Quality Improvement & Collaboration: Support the development and implementation of quality improvement initiatives across our clinics.
• Conduct chart reviews and data audits to evaluate documentation and quality measure accuracy.
• Assist with improving clinical workflows, patient outreach efforts, and care-gap reporting.
• Provide education and guidance to staff regarding quality measures and documentation requirements.
• Coordinate quality improvement projects, monitor progress, and evaluate results.
