Job summary

Family Circle of Care in Tyler, TX is seeking a Clinical Quality Improvement Analyst who will collect and analyze clinical data from EHRs and payer reports, develop dashboards, and identify opportunities to improve patient outcomes. This on-site role collaborates with clinical and operations teams.

The ideal candidate has a healthcare background, strong Excel skills, and knowledge of quality measures, UDS reporting, and PCMH standards.

Qualifications

• Experience collecting and analyzing clinical data from EHRs and reports.
• Proficiency with Excel, including formulas and pivot tables.
• Knowledge of quality measures and value-based care programs.
• Ability to communicate findings to clinical teams and leadership.
• Experience with UDS reporting or PCMH is a plus.

Responsibilities

• Clinical data analysis and reporting from EHRs and payer data.
• Develop dashboards and scorecards to track quality measures.
• Identify trends and gaps, and propose improvement strategies.
• Collaborate with clinical and IT teams to ensure data accuracy.
• Present findings to clinical teams and leadership.
• Coordinate payer incentive programs and value-based initiatives.
• Monitor quality measures and care gap reporting.
• Support UDS reporting and PCMH activities.
• Lead quality improvement projects and track results.
• Provide education on quality measures to staff.
