Job summary

Team Select Home Care in Denver, CO is seeking a Relationship Manager to coordinate care and operations for a patient population within the Long-Term Home Health/PDN division. The RM reports to the Director of Operations (DOO).

You will oversee scheduling, staffing, referrals, payroll verification, and collaboration with nursing leadership to meet financial and clinical goals while ensuring compliance with policies and PHI privacy.

Qualifications

• High School Diploma required.
• Bachelor’s degree in Business preferred.
• Minimum of 1 year management experience in healthcare (Home Health preferred).
• Proficient in Microsoft Office (Outlook, Excel, Word).

Responsibilities

• Coordinates scheduling, staffing, recruiting, and hiring to meet agency needs.
• Meets recruiting goals as assigned by DOO.
• Manages referral/intake processes including onsite visits.
• Ensures insurance eligibility and proper authorizations.
• Manages payroll verification with clinical staff.
• Supports DOO in achieving financial metrics.
• Maintains confidentiality of PHI and company information.
• Collaborates with nursing leadership and field staff to coordinate operations.
