Job summary

Optum’s At Home Healthcare in Longmont seeks a Home Health Patient Care Manager (RN) to supervise clinical services and coordinate an interdisciplinary team, ensuring high-quality home health care aligned with physician plans of care and state/federal regulations.

As a key link between patients, families, and clinicians, you will assess needs, schedule visits, manage benefits and approvals, and drive consistent, individualized care across the patient population.

Qualifications

• Current unrestricted RN licensure in state of practice.
• Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Provides clinical services within the scope of nursing practice and in accordance with the plan of care.
• Receives referrals, assigns clinicians, evaluates orders, and plans start of care visits.
• Coordinates determination of patient home health benefits, medical necessity, and insurance approvals.
• Ensures ongoing assessment and individualized care aligned with physician orders and eligibility criteria.
• Reviews assessments and plans of care daily and consults clinicians with recommendations.
• Function effectively with diverse patients regardless of age, race, creed, color, sex, disability, or financial condition.
