Job summary

Team Select Home Care in Denver, CO is seeking a Clinical Support Manager who is a registered nurse responsible for coordinating clinical activities for a designated group of patients and staff.

The role emphasizes supervision of clinicians, care coordination, adherence to standards, and participation in QAPI and compliance while supporting the DON and administration to maintain high-quality home health services.

Qualifications

• Graduate of an approved school of nursing with active RN license.
• Minimum 1 year of home health experience required.
• Undergraduate degree preferred.

Responsibilities

• Develops, supervises, and evaluates clinicians and caregivers to deliver clinical excellence.
• Assists with adherence to clinical standards and best practices.
• Coordinates patient care, referrals, and care plans across a high-volume setting.
• Participates in QAPI and compliance activities and reporting.
• Performs field visits and provides guidance to staff as needed.
• Ensures documentation and policy compliance with state health statutes.
