Team Select Home Care of Colorado, LLC is seeking a Relationship Manager to coordinate care and operations for a patient population in our Long-Term Home Health/PDN division in Denver. You will report to the Director of Operations and collaborate with nursing and field staff to ensure high-quality, compliant care.

The role emphasizes scheduling, staffing, recruiting, referrals, insurance authorizations, payroll accuracy, and achieving financial targets.

Qualifications

• High School Diploma required; Bachelor’s degree in Business preferred.
• Minimum of 1 year management experience in healthcare (home health preferred).
• Working knowledge of Microsoft Office including Outlook, Excel, and Word.

Responsibilities

• Coordinates care and operations for a patient population within a region.
• Manages scheduling, staffing, recruiting, and hiring to meet agency needs.
• Manages referral/intake process including onsite visits and insurance authorizations.
