S.T.A.R. - STAND TOGETHER AND RECOVER CENTERS, INC. seeks an Intake Specialist in Phoenix, AZ to manage referrals, schedule orientations, and support new member enrollment. You will document outreach in the EHR within 24 hours and educate potential members on available services.

Formación

• Experience providing customer service and communicating clearly with members and community.
• Ability to document referrals and enrollment steps in an electronic health record (EHR) within 24 hours.
• Knowledge of scheduling intake and orientation appointments.
• Experience explaining enrollment paperwork to new members and completing forms accurately.

Responsabilidades

• Outreach and document successful and unsuccessful attempts in the EHR for new referrals within 24 hours.
• Outreach and document ongoing attempts for members on no attendance reports.
• Provide excellent customer service in all interactions with community and potential new members.
• Educate potential members on services available.
• Schedule intake and orientation appointments for new members.
• Assist members with enrollment paperwork and ensure payer guidelines are met.
• Coordinate with center staff to communicate new member start dates and transitions into services.
• Maintain contact with clinical team members from outside agencies during admissions.
• Support development of a STAR ISP and participate in Clinical Oversights.
