Job summary

Qualifications

• Bachelor’s degree in Business Administration, Social Work, Marketing or related field.
• Associate’s degree with three years of experience.
• Healthcare navigation or admissions experience preferred.
• Knowledge of Medicare, Medicaid, Private Pay and third-party reimbursement desirable.

Responsibilities

• Coordinate pre-admission and admission paperwork with patient/family and intake sources.
• Verify patient room readiness prior to admission.
• Maintain contact with patient families and support transition.
• Provide frequent visits to new admissions for a positive patient experience.
• Orient patients to the center environment and processes.
• Participate in IDT to address navigation needs.
• Communicate admissions information to other departments.
• Develop outreach plans with partners to ensure appropriate care.
