Job summary

You will support families, liaise with referrals, and participate in the interdisciplinary team to meet patient needs.

Ideal candidates hold a relevant degree and experience in healthcare navigation, with strong communication across departments and with external partners.

Qualifications

• Bachelor’s degree in Business Administration, Social Work, Marketing or related field.
• Associate’s degree with three years experience in related field.

Responsibilities

• Coordinate pre-admission and admission processes with patients, families, and referrals.
• Verify patient room readiness before admission.
• Sustain contact with patients and families to support transitions.
• Provide frequent visits to new admissions for a positive patient experience.
• Orient the patient to the center environment.
• Participate in the center’s interdisciplinary team to assist navigation needs.
• Maintain contact with central intake within designated response times.
• Coordinate backup plans for outreach, tours and admissions for nights/weekends.
