Job summary

Abercorn Rehabilitation Center in Savannah, GA is seeking an Admissions Coordinator to support pre-admission, admission and discharge processes. You will back up the Director of Admissions, coordinate referrals, complete room assignments and ensure contracts are processed promptly.

Qualifications

• Minimum high school diploma or equivalent required.
• One or more years experience in health care education/marketing/insurance or commensurate education preferred.
• Must be familiar with medical terminology.
• Proficient in the use of personal computer.

Responsibilities

• Tracks pre-admission referral activity daily/weekly.
• Coordinates and reviews resident/patient electronic referrals and communicates with Director of Admissions to complete assessments.
• Manages the electronic referral bed-offer process and obtain pre-authorizations from HMOs.
• Validates Midnight Census with BOM.
• Conducts tours of facility with potential residents/patients and their family members.
• Assists the Director of Admissions with arrangements and confirmation of all admissions; completes room assignment and notifications.
• Welcomes new residents and ensures contracts are completed timely.
• Completes Admission Agreement with resident/patient or responsible party timely.
• Ensures specialized equipment is ordered as indicated and seeks prior approval from authorized payer.
• Ensures level of care approval obtained and/or OBRA screen (PASSAR, MIMR) is completed.
• Completes records and documentation in accordance with policy and state/federal guidelines.
• Helps resolve concerns of Resident and/or Family and participates in the Guardian Angel Program.
• Provides patient comfort by utilizing resources, transporting patients, answering calls, and reporting observations.
• Primary Back-Up for the Director of Admissions when out on PTO, FMLA or position is open.
• Training of Social Services Coordinator back-up responsibilities and monthly job-shadowing coordination.
• Receives Director of Admissions Responsibility Training and monthly job shadowing coordination.
• Protects organization’s value by keeping patient information confidential.
• Adheres to professional standards, company policies and procedures, federal, state, and local requirements.
