Eatonton Health & Rehab seeks an Admissions Coordinator to manage pre-admission and admission processes, ensuring smooth transitions for patients and families. You will verify room readiness, maintain contact with intake, and collaborate with the interdisciplinary team to navigate care needs.

Top candidates will coordinate outreach, build partnerships, and support center operations with a compassionate, service-oriented approach in a nonprofit setting.

Qualifications

• Bachelor’s degree in Business Administration, Social Work, Marketing or related field.
• Associate’s degree with three years experience.

Responsibilities

• Coordinate pre-admission and admission processes with patients and families.
• Verify patient room readiness before admission.
• Provide support to patients and families during transitions.
• Conduct regular visits to new admissions for a positive patient experience.
• Orient the patient to the center environment.
• Participate in Interdisciplinary Team to assist navigation of care.
• Maintain contact with central intake within response times.
• Coordinate back-up plans for outreach, tours and admissions for nights/weekends.
• Develop center outreach plans and activities.
• Build partnerships with healthcare partners to deliver appropriate care.
