Job summary

Oxley Park Health & Rehab in Lyons, GA is seeking a dedicated Admissions Coordinator to manage pre-admission to admission processes and coordinate with families and referral sources. You will support patient transitions and ensure a smooth entry into the center.

Work in a nonprofit, mission-driven setting with a strong team, participating in interdisciplinary collaboration and outreach activities to deliver high-quality care.

Qualifications

• Bachelor’s degree as stated or equivalent experience.
• Associates degree with 3 years in related field acceptable.
• Experience coordinating admissions and patient information.

Responsibilities

• Coordinate pre-admission and admission processes with patients and families.
• Verify patient room readiness prior to admission.
• Support patient/family transition and ongoing visits for new admissions.
• Orient patients to the center environment and care pathways.
• Collaborate with the center’s IDT for healthcare navigation needs.
• Maintain communication with central intake within designated response times.
• Develop outreach plans and partnerships with healthcare partners.
