Job summary

We are looking for a passionate and proactive Health Visitor who shares our commitment to improving health outcomes for children and families in Sutton. Main duties of the job

Join our Specialist Public Health (0-19) Service, covering Cheam and South Sutton, Carshalton, Wallington, and Central Sutton. Our Health Visiting and School Nursing teams are dedicated to improving outcomes for children and families, with a focus on school readiness, mental health and well-being, and enhancing SEND provision.

Job responsibilities

The post holder will be employed by the London Borough of Sutton (LBS) but will work as part of the Sutton Health and Care Alliance (SHC). SHC is a partnership of key local health and care providers including the Council, Epsom and St Helier's NHS Trust, South West London and St George's MH Trust, and the GP Federation. The SHC Alliance was formed to enable local health and care providers to work together to deliver high-quality health and social care for Sutton residents. The Alliance has begun delivering innovative, joined-up services, such as Sutton Health and Care at Home, launched on April 18.

The post holder will work with the Head of Service to deliver the strategic vision for children's services in the Borough, promoting interagency working between Health and Local Authority services to achieve the best outcomes for children in Sutton. The Health Visiting service is committed to providing comprehensive family health care, partnering with families to promote health within the family and community, empowering individuals to make informed health choices. Practice is underpinned by a systematic, needs-related approach, the NMC Code of Professional Conduct, reflective practice, and the four principles of health visiting:

  1. To search for health needs
  2. To stimulate awareness of health
  3. To influence policies affecting health
  4. To facilitate health-enhancing activities

Key Tasks

  • Deliver the Healthy Child Programme to Sutton's residents.
  • Facilitate early identification of health needs and interventions for children to maximize their potential.
  • Ensure all vulnerable children are identified and receive appropriate support and timely interventions.

Representative accountabilities

  • Deliver service activities and manage caseloads to meet outcomes within service standards.
  • Identify and implement improvements for continuous service enhancement.
  • Liaise with families to review needs and resolve issues.
  • Stay updated with policy, legislation, and contractual changes to ensure effective, compliant service delivery.

Budgetary accountabilities

  • Deliver the service within the available budget.

Specific accountabilities

  • Develop a child-centered public health role aligned with public health agendas, delivering health promotion activities across settings.
  • Manage caseloads responsibly as an autonomous practitioner within a team.
  • Collaborate effectively with healthcare services and participate in multidisciplinary teams supporting children's centers and extended schools.
  • Assess, plan, implement, and evaluate individual care packages, referring appropriately to ensure timely interventions.
  • Partner with GPs and immunization teams to promote immunization adherence.
  • Identify, refer, and support children with special needs, contributing to EHCP processes as per legislation.
  • Work within safeguarding policies to identify vulnerable children and contribute to interagency planning.
  • Ensure key performance indicators are met through data management and participation in service initiatives.
  • Participate in clinical governance and audit activities to maintain high standards.
  • Support early help initiatives and supervise community nursery nurses as required.
  • Support ongoing professional development and participate in safeguarding supervision.
