The role requires an unencumbered RN license and 3+ years of clinical leadership in long-term care, with proficiency in EMRs, MARs, and Office tools. Strong mentoring of staff and adherence to licensing surveys are essential.

Qualifikationen

• 3+ years of progressive clinical leadership experience in assisted living, memory care, or long-term care.
• Proficiency with EMRs, MARs, and Microsoft Office Suite.
• Strong working knowledge of state licensing survey processes.
• Experience coaching, mentoring, and building high-performing clinical teams.

Aufgaben

• Provide leadership for resident care services and clinical operations.
• Participate in resident assessments, move-ins, and service plan reviews in compliance with Mosaic policy and state regulations.
• Review resident health records, monitor weights, and ensure timely response to changes of condition.
• Maintain updated internal service plans and ensure staff implement care appropriately.
• Document key resident updates daily in the community communication log to ensure seamless coordination between care teams.
• Oversee all medication delivery systems to ensure safety, accuracy, and regulatory compliance.
• Verify physician orders, MAR transcription, and PRN parameters, ensuring clear instructions are available for Med Techs.
• Review and resolve daily missed medication reports to ensure proper administration.
