Job summary

Apploi is seeking a Health Services Manager to lead clinical operations in our California community. You will oversee resident care, conduct assessments, review plans, and ensure safe medication management in compliance with state regulations.

Ideal candidates hold an active RN license, have 3+ years of progressive leadership in long-term care, and can coach high-performing clinical teams while collaborating with care staff to maintain quality and regulatory standards.

Qualifications

• Unencumbered RN license in the state of employment.
• 3+ years of progressive clinical leadership experience in assisted living, memory care, or long-term care.
• Proficiency with EMRs, MARs, and Microsoft Office Suite; regulatory compliance.
• Strong working knowledge of state licensing survey processes.
• Experience coaching, mentoring, and building high-performing clinical teams.

Responsibilities

• Provide clinical leadership for resident care services and operations within the community.
• Participate in resident assessments, move-ins, and service plan reviews per Mosaic policy and state regulations.
• Review resident health records, monitor weights, and respond to changes in condition.
• Maintain updated internal service plans and ensure staff implement care appropriately.
• Document resident updates daily in the community log for coordination between care teams.
• Oversee all medication delivery systems for safety, accuracy, and regulatory compliance.
• Verify physician orders, MAR transcription, and PRN parameters for Med Techs.
• Review and resolve daily missed medication reports.
