Job summary

Mosaicms in Oregon City is seeking a compassionate Wellness Manager with an LPN license to lead resident care in our community. You will supervise care teams, review plans, and ensure compliant medication administration.

This full-time on-site role emphasizes collaboration, regulatory knowledge, and hands-on leadership to support residents and families while maintaining high clinical standards. Experience with EMRs is preferred.

Qualifications

• Requires high school diploma or equivalent.
• Active LPN/LVN license in good standing per state regulations.
• Minimum 2 years supervisory experience in senior living, assisted living, or healthcare preferred.

Responsibilities

• Clinical Oversight & Resident Care: oversee delivery of resident care, assessments, service plans, and progress documentation.
• Participate in move-in evaluations and ongoing assessments per Mosaic policies and state regulations.
• Review and document resident updates daily in the community log; attend service plan reviews.
• Review physician orders, MARs, and care documentation for accuracy and compliance.
• Ensure medication administration and care procedures align with regulatory and clinical best practices.
• Escalate unresolved clinical issues or medication discrepancies to RN or Executive Director as needed.
• Oversee the medication delivery system to ensure safety, accuracy, and compliance.
