Job summary

Mosaicms is seeking a Wellness Manager (No RN) in Oregon to lead clinical oversight and resident care within our senior living community. The role requires an active LPN/LVN license and at least two years of supervisory experience, with strong focus on medication management and regulatory compliance.

You will coordinate care planning, service plan reviews, and documentation, ensuring safety and quality in line with state regulations.

Qualifications

• High school diploma or equivalent is required.
• Active LPN/LVN license in good standing per state regulations.
• Minimum 2 years of supervisory experience in senior living, assisted living, or healthcare preferred.
• Strong understanding of medication management, care planning, regulatory compliance, and best practices in senior care.
• Excellent leadership, coaching, and conflict-resolution abilities.
• Proficiency with EMRs, MARs, care documentation, and Microsoft Office Suite.
• Effective communication, adaptability, and commitment to resident-centered care.

Responsibilities

• Clinical Oversight & Resident Care management and documentation.
• Oversee and support resident care, assessments, service plans, and progress notes.
• Participate in move-in evaluations and ongoing assessments per Mosaic policies and state regulations.
• Review and document resident updates daily in the community log.
• Attend initial and ongoing service plan reviews and updates.
• Review physician orders, MARs, and care documentation for accuracy and compliance.
• Ensure medication administration aligns with regulatory and clinical best practices.
• Escalate unresolved clinical issues or medication discrepancies to the RN or Executive Director as needed.
• Oversee the medication delivery system to ensure safety, accuracy, and compliance.
