Job summary

Mosaic Management is seeking a Memory Care Wellness Manager (LPN) to provide clinical oversight and resident care in our memory care community. The role focuses on assessments, service plan development, and ensuring regulatory compliance across daily operations.

The ideal candidate will have an active LPN/LVN license, at least two years of supervisory experience in senior care, and strong leadership and communication skills to guide staff and ensure resident-centered care.

Qualifications

• Active LPN/LVN license in good standing per state regulations.
• Minimum 2 years of supervisory experience in senior living, assisted living, or healthcare preferred.
• Strong understanding of medication management, care planning, regulatory compliance, and best practices in senior care.
• Excellent leadership, coaching, and conflict-resolution abilities.
• Proficiency with EMRs, MARs, care documentation, and Microsoft Office Suite.
• Effective communication, adaptability, and commitment to resident-centered care.

Responsibilities

• Clinical Oversight & Resident Care; oversee and support the delivery of resident care, including assessments and service plans.
• Participate in resident move-in evaluations and ongoing assessments.
• Review and document key resident updates daily in the community log.
• Attend and contribute to service plan reviews.
• Review physician orders, MARs, and care documentation for accuracy and compliance.
• Ensure medication administration and care procedures align with best practices.
• Escalate unresolved clinical issues or medication discrepancies to RN or Executive Director as needed.
• Medication & Health Systems Management; oversee the medication delivery system to ensure safety and compliance.
