Mosaic-Management,-Inc is seeking a Wellness Manager (No RN) to oversee clinical oversight and resident care in a supportive, collaborative community. The role emphasizes assessments, service plans, and ensuring care documentation meets state regulations.

Responsibilities include coordinating move-in evaluations, maintaining daily updates, and contributing to service plan reviews, while ensuring medication administration aligns with best practices.

Qualifications

• Active LPN/LVN license in good standing per state regulations.
• Minimum 2 years of supervisory experience in senior living, assisted living, or healthcare preferred.
• Strong understanding of medication management, care planning, regulatory compliance, and best practices in senior care.
• Excellent leadership, coaching, and conflict-resolution abilities.
• Proficiency with EMRs, MARs, care documentation, and Microsoft Office Suite.
• Effective communication, adaptability, and commitment to resident-centered care.

Responsibilities

• Participate in resident move-in evaluations and ongoing assessments per Mosaic policies and state regulations.
• Review and document key resident updates daily in the community communication log.
• Attend and contribute to initial and ongoing service plan reviews.
• Review physician orders, MARs, and care documentation for accuracy and compliance.
• Ensure medication administration and care procedures align with regulatory and clinical best practices.
• Escalate unresolved clinical issues or medication discrepancies to the RN or Executive Director as needed.
