Responsibilities

• Coordinate referrals to Care Management
• Optimize the experience of patients and families receiving hospital and ambulatory care
• Collaborate with providers and staff to identify solutions to care coordination needs
• Provide member navigational services
• Research patient concerns and provide resolutions
• Collect and document client data in the medical record
• Coordinate multiple appointments using scheduling systems
• Identify care gaps and coordinate actions to close gaps and address member needs and barriers
• Meet and accompany patients and families to initial provider appointments before and after acute and post-acute transition
• Establish collaborative relationships with physicians, payers, and healthcare team members
• Collect and communicate information to fulfill utilization and regulatory requirements
• Educate internal healthcare team members on care management and managed care concepts
• Work independently to provide excellent patient care

Requirements

• High school diploma/GED or equivalent working knowledge
• Knowledge of medical terminology
• Completion of Medical Assistant Training, Patient Care Technician Training, or Certified Nursing Assistant Training
• 2 years of health care experience
• Excellent time management and problem-solving skills
• Ability to meet constantly shifting deadlines and timelines
• Excellent customer service and recovery skills
• Bachelor's degree in Social Work preferred
• Additional related education and/or experience preferred
• Must reside in Arizona to be considered
