Job summary

Terros Health is seeking a Full-Time Health Navigator at our 27th Ave location in Phoenix, AZ. You will support patients in accessing healthcare services, focusing on MOUD, and closing gaps in care within FQHC settings.

You’ll collaborate with providers, case managers, and community resources to promote patient-centered care and improve outcomes. The role involves educating patients, coordinating appointments, and connecting patients with social determinants of health resources.

Qualifications

• High School diploma or equivalent required. Associate’s or Bachelor's in health education, social work, or a related field preferred.
• Minimum of 1.5 years behavioral health experience. Experience working with underserved populations is strongly preferred.
• Peer Support Employment Training certification preferred.
• Bilingual skills are highly desirable but not required.
• Must have valid Arizona driver’s license, be 21 years of age with minimum 3 years driving experience, and meet Terros Health’s driving policy.
• Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
• Must pass a TB Test.

Responsibilities

• Educate and advocate patients, addressing barriers to care and healthcare system questions.
• Help patients understand and adhere to care plans with potential use of lived experience.
• Provide health education and promote health literacy through one-on-one and group sessions.
• Disseminate materials on primary care, substance use, behavioral health, and wellness.
• Assist with appointment scheduling, referrals, and follow-up across services.
• Coordinate with care management teams to develop comprehensive care plans.
• Support patients in goal setting and health improvement pursuits.
• Follow up on missed appointments and re-engage patients into care.
