Job summary

Terros Health is seeking a Full-Time Health Navigator at our Olive Ave location in Glendale, AZ. The role supports patients in navigating healthcare services, with emphasis on the MOUD program and closing gaps in care within FQHCs.

The Health Navigator will collaborate with clinicians and care teams, provide education, connect to resources, and promote patient-centered care for underserved populations, including adults with serious mental illness.

Qualifications

• High School Diploma or GED with 1 year of behavioral health experience.

Responsibilities

• Educate patients and advocate for access to care.
• Coordinate appointments, referrals, and follow-up care across services.
• Connect patients to community resources and social determinants of health.
• Follow up on calls and support patients throughout their care journey.
