Qualifications

• High School diploma or GED required.
• At least 1.5 years of behavioral health experience.
• Peer Support Employment Training certification preferred.
• Bilingual skills desirable but not required.
• Must have a valid Arizona driver's license and meet Terros Health driving policy.
• Must pass TB test and obtain Arizona fingerprint clearance.

Responsibilities

• Educate patients and advocate for access to care.
• Coordinate appointments, referrals, and follow-up across services.
• Connect patients to community resources and social determinants of health.
• Follow up on missed appointments and support treatment adherence.
