Job summary

Terros Health seeks a Full-Time Health Navigator at the 27th Ave location in Phoenix, AZ. The Health Navigator will help patients access and navigate healthcare services with emphasis on MOUD and closing gaps in care.

The role works with case managers, providers, and community resources to support underserved populations. Responsibilities include education, advocacy, and care coordination, plus assisting patients in reaching health-related goals and following up on care plans.

Qualifications

• Minimum High School Diploma or GED with 1 year of behavioral health experience.
• Ability to educate, advocate, and support patients navigating care.
• Strong communication and coordination with care teams and community resources.

Responsibilities

• Educate patients and advocate for access to care and resources.
• Coordinate appointments and follow‑up across primary and behavioral health services.
• Connect patients to social determinants of health resources (housing, transportation, finances).
• Follow up with patients to ensure understanding and adherence to care plans.
