Job summary

Terros Health in Phoenix, AZ is seeking a Full-Time Health Navigator to support patients at our McDowell Rd location within FQHC settings, focusing on MOUD program access and closing care gaps. You will educate, advocate and coordinate care for adults including those with serious mental illness and underserved populations, working with care teams to improve health outcomes.

The role requires a high school diploma with 1+ year behavioral health experience, bilingual skills desirable, and a valid

Qualifications

• High School Diploma or GED in combination with one (1) year of behavioral health experience.
• 1.5 years behavioral health experience preferred.
• Bilingual skills are highly desirable but not required.
• Must have a valid Arizona driver’s license and 3+ years driving experience per Terros Health policy.
• Arizona Fingerprint Clearance card or able to apply within 7 working days of starting.

Responsibilities

• Educate patients and advocates for access to healthcare services.
• Coordinate with care teams to schedule appointments and referrals.
• Assist with MOUD program navigation and closing gaps in care.
• Connect patients to community resources addressing housing, transportation, and financial needs.
• Follow up on missed appointments and re-engage patients into care.
