Job summary

Terros Health is seeking a Full-Time Health Navigator in Mesa, AZ, to support patients at our Stapley Dr location. The role focuses on MOUD navigation, patient education, and closing gaps in care within our FQHC network.

You will collaborate with providers, case managers, and community resources to promote patient-centered care, assist with scheduling and referrals, and help patients access essential health services. Full benefits and a daytime schedule are offered.

Qualifications

• High school diploma or GED with 1 year of behavioral health experience.
• Experience navigating health systems and coordinating care.
• Ability to educate patients and promote health literacy.

Responsibilities

• Educate, advocate, and outreach to patients.
• Coordinate appointments, referrals, and follow-up across primary care and behavioral health services.
• Connect patients with social determinants of health resources and community partners.
• Follow up with patients and ensure timely, team-based responses to inquiries.
