Job summary

Terros Health seeks a Health Navigator for its 27th Ave location in Phoenix, AZ. This full-time on-site role supports patients in navigating care within Federally Qualified Health Centers, with a focus on the MOUD program and closing gaps in care.

The navigator educates, connects resources, and coordinates services to enhance outcomes. The ideal candidate has a high school diploma or GED with 1.5+ years of behavioral health experience and a valid Arizona driver’s license.

Qualifications

• High School Diploma or GED with 1 year of behavioral health experience.
• 1.5+ years behavioral health experience preferred.
• Bilingual skills desirable but not required.
• Must have valid Arizona driver’s license and be 21+ with 3+ years driving experience; comply with Terros Health driving policy.
• Arizona Fingerprint Clearance card or apply within 7 working days.

Responsibilities

• Educate patients and advocate for access to care; promote health literacy.
• Coordinate care across primary care, behavioral health, and specialty services.
• Assist with appointment scheduling, referrals, and follow-up care.
• Connect patients to community resources addressing housing, transportation, and other social determinants of health.
• Follow up on missed appointments and re-engage patients into care.
